ENROLLMENT INFORMATION
The lottery procedure was held on (1/16/2024), we enrolled all new students first come first serve until we fill our capacity.
For enrollment: Please visit the school website for detailed lottery procedures and dates.
- You can come to our school office to receive and complete the enrollment form.
- You can download an Enrollment Form from the right column of our website and complete and send it with the documents.
- You can complete an Online Enrollment Form from our website, then we will contact you for signatures and documents.
- Please bring the following documents to finalize your enrollment:
- Proof of birth: Birth Certificate, Baptismal Certificate, Passport (Please see AB 699 Compliance)
- Parent's photo ID: Guardianship documents if you are not the parent.
- A document that shows the address: Utility bill (DWP, Gas, just for documentation purposes)
- Physical Exam(for new to country or state students), vaccination record, and TB test
- The latest report card
- A recent copy of IEP (if it is applicable)
- GATE program letter of the document (if it is applicable)
- Please bring the following documents to finalize your enrollment:
Shots Required for TK/K-12 Students
Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) - 5 doses (4 doses OK if one was given on or after the 4th birthday. 3 doses OK if one was given on or after the 7th birthday.)
Polio (OPV or IPV) - 4 doses (3 doses OK if one was given on or after 4th birthday)
Hepatitis B - 3 doses (not required for 7th-grade entry)
Measles, Mumps, and Rubella (MMR) - 2 doses (Both given on or after 1st birthday)
Varicella (Chickenpox) - 2 doses
These immunization requirements also apply to students entering transitional kindergarten.
California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child's Immunization Record as proof of immunization.

AB 699 Compliance
Dear Parents/Guardians,
If for any reason you don’t have your child's Birth Certificate or don't want to disclose your child’s birth country, you will be able to provide that information later when completing the “Students Information Update Sheet” during the school year. Completing this information helps your child’s school secure additional funding to support its educational program.
