Starting January 14, 2022, we are enrolling
all students first come first serve until we fill our capacity.

Ways to Enroll

To enroll your student:

You can come to our office to receive and complete the enrollment form.

You can download the ENROLLMENT FORM and send it to the school office with the documents.

You can complete an Online Enrollment Form, then we will contact you to complete the enrollment.

Documents for enrollment

  • Proof of birth: Birth Certificate, Baptismal Certificate, Passport (Please see AB 699 Compliance)
  • Parent’s photo ID: Guardianship documents (if you are not the parent by Birth Certificate)
  • Proof of address: Utility bill (DWP, Gas)
  • Physical Exam (for students new to country or state)
  • Vaccination record, and TB test results (Important )
  • Current report card (optional)
  • A recent copy of IEP (if applicable)
  • GATE program letter of the document (if applicable)

REQUIRED SHOTS

Tetanus, Diphtheria, Pertussis (Tdap) —1 dose
(Whooping cough booster usually is given at 11 years and up)

Varicella (Chickenpox) — 2 doses
(Usually given at ages 12 months and 4-6 years)

California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization.

AB 699 Compliance

Dear Parents/Guardians,

If for any reason you don’t have your child’s Birth Certificate or don’t want to disclose your child’s birth country, you will be able to provide that information later when completing the “Students Information Update Sheet”  during the school year. Completing this information helps your child’s school secure additional funding to support its educational program.
